Our nurses, RNs and LPNs, are

direction of a provider, they
provide outstanding medical care,
behavioral health support and case
management to our patients.

• Coordinate care between interdisciplinary team

• Vital signs, respiratory and cardiac oversight

• Home safety support

• Nutritional education

• Train on required medical equipment

• Injections

• Wound and post-procedural care

MEDICATION ADMINISTRATION

Our nurses help to ensure the secure and safe delivery of medications.

• Coordinate with pharmacy around medication delivery

• Communicate with providers about refills, side effects, and patient’s responses to medications

• Secure delivery of medications as needed

OCCUPATIONAL THERAPY

Our Occupational Therapists, OTs and COTAs, focus on adapting the environment to the patient and assisting them in overcoming barriers affecting their emotional, psychosocial, and physical needs.

• Develop strategies to manage anxiety, depression, and stressors

• Fine motor skills and eye-hand coordination

• Cognitive skills (motor planning, sequencing, organization)

• Improve strength and endurance

• Self-advocacy skills

• Activities of daily living including home management tasks and meal preparation

• Visual perception skills

• Establish healthy roles and routines

HOME HEALTH AIDE

Our Home Health Aides are trained and certified healthcare workers who provide assistance in a patient’s home. They are under the direct supervision of a Registered Nurse.

• Bathing and dressing

• Light housekeeping (laundry, vacuuming)

• Meal planning and preparation

• Companionship visits
