Relief Home Home Health Services Inc. was founded nearly 15 years ago to provide compassionate medical and psychiatric care through a highly trained, multidisciplinary team. With a robust staff of RNs, LPNs and Occupational Therapists, we have established ourselves as a leader in the home-based healthcare industry in Massachusetts. We currently care for adult patients throughout Cape Cod and in Bristol County. Relief is accredited by the highly regarded CHAP (Community Health Accreditation Partner) and CMS (Center for Medicare and Medicaid Services). Leaning back on our mission, Relief serves all individuals in our community, focusing on the underserved with innovative and client-centered approaches. We strongly believe that patients are best supported in the comforts of their own homes.

Our Mission Statement
Relief Home Health Services provides comprehensive, highly skilled, and compassionate medical care for patients challenged with behavioral health and medical comorbidities in our community. Each patient is treated as an individual and given the tools to take an active role in their healthcare. We empower and support our employees through ongoing training and educational opportunities. In collaboration with our community partners, we reduce Emergency Department visits and Hospital Admissions for the underserved population.

Skilled Nursing
Under the direction of a physician, our highly skilled and compassionate nurses provide outstanding medical care, emotional and psychiatric support, and case management for patients and their families from the comfort of their homes. Visit frequency is established based on the patient's needs.

Occupational Therapy
Our Occupational Therapists work with physical, sensory, and cognitive problems to help patients work past barriers that affect their emotional, social, and physical needs. They advise patients on developing, recovering, and maintaining the skills needed for success in their daily lives.

MEDICATION MANAGEMENT
Our nurses help to ensure the secure and safe delivery of medications. In collaboration with providers, they communicate about refills, side effects, and general patient response to medications to promote medication adherence which has been shown to benefit overall health outcomes.
