OUR Community CoLLABORATORS

We work closely with nonprofit, healthcare and medical-based organizations on Cape Cod and in New Bedford. Our community partnerships allow for innovative and customized care solutions that help to meet the needs of marginalized groups in our community. Many partnerships make care more accessible for homeless, women's health, migrant and those dealing with mental illness.

Duffy Health Center

In collaboration with Duffy Community Health Center we have helped to bring care directly to those dealing with homelessness or housing insecurity in the Cape Cod Community.

Health Imperatives

In collaboration with the team at Health Imperatives, we helped to coordinate to bring reproductive health care to those temporarily housed in EA Shelters on the Cape and Southeastern MA.

NAMI

In collaboration with the National Alliance on Mental Illness, we have been able to respond to the mental health needs of those in our community with nursing care.

Whole Health Pharmacy

We have a close, collaborative relationship with Whole Health Pharmacy. Their team understands the unique needs of many of our patients and prepares medications to set patients up for success. We have also worked closely with Whole Health on Covid-19 testing sites to meet testing demands and community needs during the pandemic.

Apothecare

Apothecare is a pharmacy in Hyannis. The response time for requested medications has been phenomenal. We have a very strong, collaborative relationship with them and are able to break through obstacles together.

Eterna Pharmacy

Eterna is always willing to do everything they can to meet patient needs. If a patient isn't home for a delivery, Eterna will do their best to track down a Relief nurse to help with medication delivery. They always go the extra mile in collaborating with us for our patients.
